On Friday August 10, 2018 at 4 pm, Mr. Abdoulaye Diouf SARR, Minister of Health and Social Action (MSAS) and the administrative and municipal authorities of the Commune of Yeumbeul inaugurated the ARISTIDE MENSAH Integrated Community Health and Development Center located at Cité COMICO 4 in Yeumbeul. The project, made a reality thanks to the vision of the Institut de la Formation et de la Recherche en Urologie et Santé Familiale (IFRU-SF) chaired by Professor Serigne Magueye GUEYE, has benefited from the support of institutional and individual partners, some of whom remain anonymous.

In his speech, the Minister of Health said he was heartened by the presence of the minimum package of services, coupled with consultations in specialties such as Urology, Gynecology and Pediatrics. According to Mr. Abdoulaye Diouf SARR, "initiatives such as these help to build a resilient healthcare system. A system that will ensure equity in the provision of healthcare services to communities".

Mr. Daouda NDIAYE, Mayor of the Commune of Yeumbeul Nord, expressed his satisfaction at seeing such an infrastructure built in his area. He expressed his delight at the expanded range of care on offer, which helps to make quality care financially and geographically accessible, at a cost that the people of Yeumbeul can largely afford.

In 2020, 619 million people suffered from lower back pain globally.

By 2050, 843 million people are projected to fall into this category.

Risk factors are work-related ergonomic factors, obesity, and smoking.

Study shows low back pain increases with age and the peak age impacted is 85.

Globally, low back pain is more prevalent in women than men in all age groups.

Regions with the highest occurrence of low back pain are Central Europe, Eastern Europe, and Australasia.

Increase in low back pain cases will be driven by population growth except in Asia and Latin America, where the main driver will be an aging population.

A new study shows low back pain remains the leading cause of disability globally. In 2020, low back pain impacted 619 million individuals. That number is expected to increase in the next three decades to 843 million. The peer-reviewed study published today in

, was conducted by the Institute for Health Metrics and Evaluation (IHME) at the University of Washington’s School of Medicine as part of the Global Burden of Disease Study 2021.

The study shows older people are particularly at risk as the prevalence of low back pain increases with age. The peak impacted age is 85 years.

“From a health care perspective, clinical practice guidelines in most countries lack specific recommendations on how to care for an older person with low back pain. The elderly are the most vulnerable population to this condition,” says first author Prof. Manuela Ferreira, Global Burden of Disease study collaborator and Professor at Sydney Musculoskeletal Health, at the University of Sydney.

In 2020 globally, low back pain caused a total of 69 million years lived with disability (YLDs), a measurement of years lived in less-than-ideal health. This was a decrease from 1990 in the percentage of all-cause YLDs, but low back pain remained the main driver of YLDs globally.

The main risk factors for low back pain identified in the study are smoking, obesity, and occupational ergonomic factors – such as repetitive, fast movements or handling heavy loads at work. These risk factors make up almost 40% of years lived with disability within low back pain cases.

The number of low back pain cases varies by geographic region, causes, and age. The study covers local, regional, and national data from 1990 to 2020 in 204 countries and territories. The nations with the highest age-standardized rates of low back pain are Hungary and the Czech Republic. The countries with the lowest rates are Myanmar and the Maldives. The data suggest that the rate of population growth and aging in all countries in the study may have impacted their rankings.

By 2050, the research projects a 36.4% increase to 843 million people impacted by low back pain, with the largest rise in Asia and Africa. Most of the increase will be driven by population growth except in East Asia, South Asia, Latin America, and the Caribbean, where aging is expected to be the main driver.

“The societal and economic impact from low back pain is enormous. We see loss of productivity from missed days at work and a reliance on medication to alleviate pain,” says Dr. Jaimie Steinmetz, managing research scientist at IHME. “The research shows the need for more public health prevention strategies, especially for the elderly. For working-age populations, low back pain forces more people out of the workplace than any other chronic health condition.”

The global data in the study can help policymakers and health care providers develop or improve prevention strategies targeting risk factors of ergonomics, obesity, and smoking

Prostate cancer (PCa) is the leading non-cutaneous cancer among American men1. African Americans (AA) suffered from the highest incidence rates of prostate cancer (PCa) in the world, with an annual incidence rate of 165/100,000 in the period from 2004 to 2008 and 229 / 100,000 in the period from 2006 to 20102,3. The GLOBOCAN program of the International Agency for Research on Cancer (IARC) estimates that PCa remains the leading cancer in terms of incidence and mortality among men in Africa and the Caribbean4. Consequently, men of African origin scattered around the world suffer disproportionately from CaP compared to men of other races or ethnicities 5.

The IARC also estimates that CaP is in an upward trend in Africa with the number of deaths from CaP which will increase from 55,522 in 2010 to approximately 105,758 by 20301, i.e. double the number of cases that will be diagnosed in the United States. in 2030. Additionally, of all ethnic groups in the United States1, AA men have the highest rates of aggressive CaP and CaP-specific mortality. Our understanding of this disparity is very limited. In sub-Saharan Africa (SSA), the majority of CaP cases are diagnosed with aggressive disease, often at an advanced stage and generally incurable6,7. In both SSA and AA men, this situation could result from a combination of biological aggressiveness and late detection. Thus, there may be common features in the etiology of CaP in men of African origin that may explain the observed mortality values. Knowledge gained from studies of CaP in SSA could improve our understanding of CaP aggressiveness in men of African origin located worldwide.

Despite the clear public health implications of CaP in men of African descent, there have been few established associations between exposure to genetic susceptibility loci and environmental factors with the etiology of CaP. in AA. On the other hand, CaP is, among major cancers, the one with the highest heritability8,9. Numerous genetic susceptibility loci have been identified in men of European and Asian origin. Also, several genome-wide association studies (GWAS) have been reported, but none focusing exclusively on African populations. Most of the loci identified by GWAS were not reproduced in AA. Thus, there is a pressing need to identify specific African alleles and, thereby, elucidate the etiology of CaP in AA men. Furthermore, the development of infrastructure that can address the genetics of CaP in Africa will help build research capacity in the continent to widely promote cancer research.

           SPECIFIC OBJECTIVES

To better understand the etiology of CaP in African men, we have established a large multicenter consortium entitled “Men of African ancestry and prostate cancer” (MADCAP). Using the resources of this consortium, we propose to undertake a multicenter study of CaP in SSA pursuing the following objectives:

           Specific objective 1:

Genetic susceptibility: Discover new CaP loci and validate previously known CaP loci in African men to provide new information on the genetic etiology of CaP.

Goal 1.1: Undertake a genome-wide association study on the etiology of CaP and its aggressiveness in a set of pan-African samples representative of the ancestral origin of African-Americans.

           Specific objective 2:

Population Genomics: Assess how population differentiation and the recent evolutionary history of African and African-American populations may shed light on the reasons underlying high rates of CaP among African-Americans.

Aim 2.1: Identify variants with large differences in allele frequency between continents and populations (including ancestry informative markers and AIMS) and assess whether these variants correlate with increased CaP risk and aggressiveness of the disease. CaP among African and African American men.

Aim 2.2: Assess how the evolutionary history of different genomic regions, taking into account genetic “hitchhiking” between CaP susceptibility alleles and locally adaptive alleles at closely linked loci, might contribute to differences in risk of CaP in men of African origin.

The signing of the MOU on Wednesday, April 26, 2023, by leadership of Doylestown Health and a delegation from Senegal (pictured below), represents the next phase of growth in a years-long partnership between Albert Ruenes, Jr., MD, Doylestown Health Urology, and Senegalese colleagues. The MOU establishes clinical cooperation and support between the medical centers and Doylestown Hospital and will support the training and exchange of clinical information and skills between physicians and other clinicians, among other educational priorities.

“We are grateful for the work of Dr. Ruenes and his Senegalese colleagues in paving the way for our institutions to come together in this partnership. Even though we are thousands of miles away, our missions are the same: to put patients first and improve the health and well-being of the people we serve,” says Jim Brexler, President and CEO of Doylestown Health.

In 2004, Dr. Ruenes established ASSISTS (America-Senegal Surgical Initiative, Surgeons Teaching Surgeons), a non-profit organization that provides African surgeons training and support to help eliminate suffering from medical conditions endemic to West Africa. The collaboration involves American and West African surgeons working together in Senegal to incorporate contemporary surgical techniques and improve the facilities in which these services are delivered.

“I’ve explored both the challenges and great rewards of delivering health care to rural indigent populations. It is not only the patients and small communities who benefit but also the clinicians delivering the care. Humanitarian work teaches empathy, humility, and reinforces the fundamental ethical standards we commit to when taking the Hippocratic Oath. Over the years, it’s been my honor to be welcomed with such warm hospitality by my Senegalese partners, and I’m delighted that I could introduce them to my colleagues at Doylestown Hospital this week. It’s been a wonderful experience for all of us,” says Dr. Ruenes.

A dinner and speaking program followed. Scott Levy, MD, Vice President and Chief Medical Officer of Doylestown Health, paid tribute to the work of ASSISTS, and shared, “If anything personifies the work of a humanitarian, it is the work that Dr. Ruenes is doing, and we are very grateful.” Dr. Ruenes and colleague Dr. Kurt McCammon presented Dr. Serigne Magueye Gueye with the prestigious Humanitarian of the Year award. Dr. Gueye was joined by his physician colleagues, and associates from Centre de Santé Aristide Mensah in Yeumbeul, Dakar, Senegal.
